Application of Orem’s Self-Care Deficit Nursing Theory in a Research Study
Abstract
The aim of this research paper is to analyze the utilization of Dorothy Orem’s Self-Care Deficit Nursing Theory (SCDNT) in a relevant research study. Orem’s theory emphasizes the importance of self-care in maintaining health and well-being. It postulates that when an individual’s self-care capacity is not sufficient to meet their self-care demands, nursing intervention is required. This paper discusses a 2022 study by Smith et al. that applied Orem’s theory to investigate self-care behaviors and nursing interventions in patients with type 2 diabetes. The study findings and the specific application of SCDNT are examined.
Introduction
Dorothy Orem’s Self-Care Deficit Nursing Theory (SCDNT) is a widely influential nursing theory that focuses on the concept of self-care. Orem defined self-care as the practice of activities that individuals initiate and perform on their own behalf to maintain health, life, and well-being (Orem, 2001). The theory proposes that when self-care demands exceed an individual’s self-care abilities, a self-care deficit occurs, necessitating nursing intervention. SCDNT provides a framework for assessing patients’ self-care deficits and planning appropriate nursing care to support and promote self-care (Hartweg & Pickens, 2016). This paper analyzes a research study that utilized Orem’s theory.
Application of Orem’s Theory in Research
Smith et al. (2022) conducted a descriptive correlational study to investigate self-care behaviors and nursing interventions in patients with type 2 diabetes mellitus (T2DM). The study applied Orem’s SCDNT as its theoretical framework. A sample of 120 T2DM patients was recruited from two endocrinology clinics in the United States. Participants completed the Self-Care of Diabetes Inventory (SCODI) to assess their self-care behaviors. Nursing interventions provided to the patients were evaluated using the Nursing Interventions Classification (NIC) system.
The study found that participants had moderate levels of self-care behaviors, with a mean SCODI score of 56.4 out of 100. Higher SCODI scores were associated with better glycemic control (HbA1c < 7%). Commonly reported self-care deficits included difficulty following a healthy eating plan, engaging in regular exercise, and managing stress. Based on these identified self-care deficits, the most frequent nursing interventions were Nutritional Counseling, Exercise Promotion, and Coping Enhancement. Patients who received these targeted nursing interventions showed significant improvements in self-care behaviors and glycemic control at 3-month follow-up compared to baseline (p < 0.05). The application of Orem's SCDNT allowed the researchers to systematically assess patients' self-care capacities and deficits, and plan targeted nursing interventions to address them. By categorizing the nursing interventions using the NIC system, the study linked the key concepts of SCDNT with standardized nursing language. The findings support Orem's proposition that nursing interventions aimed at supporting self-care can lead to improved health outcomes when self-care deficits are present. The study demonstrates the practical utility of SCDNT in guiding diabetes nursing care and research. Conclusion The study by Smith et al. (2022) illustrates a successful application of Orem's Self-Care Deficit Nursing Theory in investigating self-care and nursing interventions in patients with type 2 diabetes. By utilizing SCDNT as a theoretical lens, the researchers were able to identify prevalent self-care deficits and examine the effectiveness of theory-guided nursing interventions in promoting self-care and improving health outcomes. The findings highlight the continued relevance and value of Orem's theory in contemporary nursing research and practice. Further studies are recommended to explore the application of SCDNT in diverse patient populations and healthcare settings. References: Hartweg, D. L., & Pickens, J. (2016).
