Psychosocial Tool Selection: For this QI project, the psychosocial tool selected is the Headache Impact Test-6 (HIT-6). The HIT-6 is a self-administered questionnaire that assesses the impact of headaches on the quality of life of patients. It consists of 6 items that cover the impact of headaches on pain intensity, social functioning, role functioning, vitality, cognitive functioning, and psychological distress. The HIT-6 has been found to be a reliable and valid instrument for measuring the impact of headaches on patients’ quality of life.

Outcome Measures: The outcome measures selected for this QI project are:

The proportion of patients who have a reduction in the frequency and intensity of headaches after receiving treatment
The mean change in the HIT-6 scores of patients before and after treatment
The proportion of patients who are satisfied with their headache management
The proportion of patients who receive appropriate referral to a specialist for further management of their headaches
Nurse Sensitive Outcome Measures: The nurse sensitive outcome measures selected for this QI project are:
The proportion of patients who have received education on headache management and preventive measures
The proportion of patients who have received appropriate medication education and instructions on medication adherence
The proportion of patients who have received appropriate follow-up care and monitoring of their headaches
Outcome Assessment Strategies and Evaluation Methods: The following assessment strategies and evaluation methods will be used to measure the selected outcomes:
Patients will be asked to complete the HIT-6 questionnaire before and after treatment to measure the impact of headaches on their quality of life.
A chart audit will be conducted to collect data on the proportion of patients who have received education on headache management, medication education, and follow-up care.
Patient satisfaction surveys will be administered to assess their satisfaction with headache management.
Electronic health record data will be used to track the proportion of patients who have received appropriate referral to a specialist.
Tools: Attached is a copy of the Headache Impact Test-6 (HIT-6) questionnaire.
Overall, the selected psychosocial tool and outcome measures will provide valuable information on the impact of headache management on patients’ quality of life, as well as the effectiveness of the interventions implemented. The nurse sensitive outcome measures will ensure that patients receive appropriate education, medication instructions, and follow-up care to optimize their outcomes.

For the Instrument and Metric for QI Plan Assignment, you will continue to build upon this work by selecting a psychosocial tool and discussing methodology to measure outcomes.
