Omeprazole (Prilosec) is a proton pump inhibitor (PPI) commonly prescribed to reduce stomach acid production and treat conditions such as gastroesophageal reflux disease (GERD), ulcers, and Zollinger-Ellison syndrome (Hershman & Koch, 2018). Upon oral administration, omeprazole is rapidly absorbed and distributed systemically (Hershman & Koch, 2018). It is metabolized in the liver by cytochrome P450 enzymes and has a plasma half-life of 0.5-1 hour (Hershman & Koch, 2018).
Omeprazole works by irreversibly blocking the hydrogen-potassium adenosine triphosphatase enzyme (the proton pump) located on the surface of gastric parietal cells (Hershman & Koch, 2018). This inhibits the final step of acid production, resulting in a profound and long-lasting reduction in gastric acid secretion (Hershman & Koch, 2018). The pharmacodynamic effect allows for once or twice daily dosing for the treatment of acid-related gastrointestinal disorders.
Patients prescribed omeprazole should take it exactly as prescribed, usually 30-60 minutes before eating. Common side effects may include headache, diarrhea, nausea, flatulence, and abdominal pain (Hershman & Koch, 2018). Patients should avoid taking omeprazole with antacids or other PPIs, as this can decrease its effectiveness. Omeprazole may also interact with certain medications metabolized by CYP450 enzymes. Patients should notify their provider of all medications and supplements taken to avoid potential drug interactions. Proper medication adherence is important to control acid-related conditions.
