Exploring the Concept of Patient-Centered Care in Advanced Nursing Practice

The concept of patient-centered care has emerged as a fundamental principle in advanced nursing practice. This approach prioritizes the individual needs, preferences, and values of patients and their families in the delivery of healthcare services (Santana et al., 2018). The purpose of this paper is to explore the concept of patient-centered care, discuss its application in nursing practice, and consider the knowledge and skills required to achieve related goals.

The Concept of Patient-Centered Care in Nursing
Patient-centered care is widely used in various aspects of nursing, including practice, research, and theory development. In practice, nurses adopt this approach by actively involving patients in decision-making, respecting their autonomy, and providing individualized care (Delaney, 2018). Nursing research focused on patient-centered care aims to identify best practices and evaluate the effectiveness of interventions that prioritize patient preferences (Jakimowicz & Perry, 2019). Achieving Patient-Centered Care Goals
To achieve patient-centered care goals, nurses must possess a comprehensive understanding of the concept and its application in practice. This knowledge includes recognizing the unique needs and preferences of each patient, effectively communicating with patients and their families, and collaborating with interdisciplinary teams to provide coordinated care (Santana et al., 2018). Nurses may need to develop additional skills, such as cultural competence and empathy, to deliver truly patient-centered care (Delaney, 2018).

Conceptualizing Patient-Centered Care
Patient-centered care can be viewed as both a process and an outcome. As a process, it involves the implementation of specific strategies and interventions that prioritize patient preferences and values (Jakimowicz & Perry, 2019). As an outcome, patient-centered care is reflected in improved patient satisfaction, enhanced therapeutic relationships, and better health outcomes (Wei et al., 2019).

The Nurse’s Role in Managing Patient-Centered Care
Nurses play a critical role in the management of patient-centered care. They are responsible for assessing patient needs, developing individualized care plans, and advocating for patient preferences (Santana et al., 2018). Nurses also serve as key communicators, facilitating the exchange of information between patients, families, and healthcare providers (Delaney, 2018). By embracing the concept of patient-centered care, nurses can significantly contribute to the delivery of high-quality, personalized healthcare services.

Conclusion
Patient-centered care is a vital concept in advanced nursing practice, emphasizing the importance of prioritizing patient needs, preferences, and values. Nurses must possess the knowledge and skills necessary to effectively implement patient-centered care strategies and achieve related goals. The Concept Map assignments are a progressive learning activity that builds with each assignment. This will then be used, along with a systems theory, in your final paper assignment.

Some weekly assignments will ask you to submit a rough draft of your concept map work to show your progress and keep you on track. In week 9, you will post your concept map with a brief explanation to the discussion board for peer review. The final Concept Map will be submitted along with your Final Paper. The final paper is a combination of the concept map assignment you complete during the course.
