You are required to work as part of a team to plan, deliver and evaluate a health promotion activity and produce an individual report based on your chosen area of health promotion and social and demographic changes. The report should be 2500 words (+/- 10%).

Explain the impact on healthcare provision of demographics and social change. Demography is concerned with the study of human populations, their structure and change (through births, deaths, and migration), and their relationship with the natural environment and with social and economic change. You should consider the intensifying pressures on health systems. (1.1)

When evaluating, consider the impact of social change, in relation to healthy lifestyle and life expectancy. You need to be able to comment on the impact and effect that the developments have had upon health. You need consider the strengths and limitations (the positive and negative) aspects of these social changes. (1.2)

Models of health promotion
In your report you need to explain three different models of health promotion (2.1) You should identify each model and explain how each approach is different in delivering information to a target audience. You should indicate which model of health promotion is relevant to your activity. You should also analyse the role of the health professional in delivering health promotion campaigns. (2.2)

Plan the health promotion activity.
You must produce a group plan for your activity. (Include in your appendices). These plans must be easy to follow and can be in the form of a table or mind map. There is a template available for your group plan (3.1)

Select an area where you think health promotion is required. You should introduce the topic area and target audience for this activity. The report must justify the plans you have produced addressing the reasons for choosing the topic and what you believe the intended outcomes for your activity will be. (3.2). You must identify ethical issues within your activity (3.3), examples will be discussed in your classroom sessions.

Health Promotion Activity
All members of the group must actively undertake the health promotion activity. Your activity needs to be effective, engaging and interactive. Activities and resources will need to be developed by each group. Visual aids will need to be produced as you will decorate your own stands for this event (4.1)

Evaluation and Recommendations
You need to evaluate the activity using an evaluation tool that you have designed such as a short questionnaire. Evaluate your activity focusing upon the value and limitations of your health promotion activity. Suggest recommendations to show how you could improve this health promotion activity if it were to be repeated. (4.2)

Appendices (if used) and a reference list must be attached at the end of your report. You will need to include evidence of your participation in the group activity; this should form part of your appendices in the form of photographs. Note: A photographic consent form will also need to be completed and signed as you may have your images used for college purposes.
