1. Assess the patient’s current situation and history, including their diagnosis, symptoms, medication, treatment goals, strengths and challenges. Use a variety of sources such as interviews, observations, records and standardized tools to gather information.
2. Identify the patient’s ALs that are affected by their mental health condition and how they cope with them. For example, a patient with depression may have difficulties with communication, eating and drinking, working and playing and sleeping. A patient with schizophrenia may have problems with maintaining a safe environment, breathing, elimination and sexuality.
3. Determine the patient’s level of dependence or independence on each AL using a continuum scale from 0 (complete dependence) to 5 (complete independence). Consider the five factors that influence the ALs and how they affect the patient’s ability to perform them. For example, a patient with anxiety may have a low level of independence on communication due to psychological factors such as fear and low self-esteem.
4. Plan interventions that are tailored to the patient’s needs and preferences on each AL. The interventions should aim to enhance the patient’s independence, quality of life and recovery. They should also be evidence-based, realistic, measurable and time-bound. For example, an intervention for a patient with bipolar disorder who has difficulty with sleeping may be to provide education on sleep hygiene, monitor their sleep patterns, adjust their medication regimen and encourage relaxation techniques before bedtime.
5. Implement the interventions in collaboration with the patient, their family and other members of the multidisciplinary team. Provide support, guidance and feedback to the patient throughout the process. Document the interventions and their outcomes in the care plan.
6. Evaluate the effectiveness of the interventions on the patient’s ALs and their mental health status. Review the care plan regularly and make adjustments as needed based on the patient’s progress, feedback and changing needs.

The Roper-Logan-Tierney model is a useful tool for preparing care plans for psychiatric patients as it provides a comprehensive and holistic approach that focuses on the patient’s individual needs and strengths. It also helps nurses to identify potential problems and risks, prioritize interventions and monitor outcomes.
