Is Increased Life Expectancy a Burden on the Healthcare System?

The world’s population is aging rapidly, and this has profound implications for the healthcare system. According to the World Health Organization, the number of people aged 65 and older will reach 1.5 billion by 2050, accounting for 16% of the global population. This demographic shift is driven by improvements in living standards, sanitation, nutrition, and medical care, which have reduced mortality from infectious diseases and increased life expectancy. However, longer lives also mean more years of living with chronic and degenerative conditions, such as cancer, diabetes, cardiovascular disease, dementia, and osteoporosis. These conditions require more intensive and costly healthcare interventions, which pose significant challenges for health systems in terms of financing, workforce, infrastructure, and quality.

The question of whether increased life expectancy is a burden on the healthcare system is not straightforward to answer. On the one hand, there is evidence that aging populations increase the demand for healthcare services and expenditures. For example, a study by Zarulli et al. (2021) found that health expenditure per capita increases by 0.7% for every additional year of life expectancy at birth . Similarly, a report by Promedcert (2023) estimated that cancer cases will rise by 50% by 2030 due to the aging population, requiring more resources for oncology research and treatment . Moreover, elderly people tend to have multiple comorbidities and complex needs that require coordinated and integrated care across different levels and sectors of the health system.

On the other hand, there are also arguments that increased life expectancy is not necessarily a burden on the healthcare system, or at least not as much as commonly assumed. For instance, some studies have suggested that proximity to death, rather than age per se, is a more important determinant of health expenditure. This means that people who live longer may not incur higher healthcare costs over their lifetime, but rather compress them into a shorter period before death . Furthermore, some researchers have pointed out that aging populations may also bring positive effects for the health system, such as increased productivity and innovation, intergenerational solidarity and support, and greater demand for preventive and wellness services .

Therefore, the impact of increased life expectancy on the healthcare system depends on various factors, such as the quality of life of the elderly population, the availability and accessibility of healthcare services, the efficiency and equity of health financing mechanisms, and the adaptability and resilience of health policies and institutions. Rather than viewing increased life expectancy as a burden or a threat, it may be more productive to see it as an opportunity and a challenge to improve the health system’s performance and responsiveness to the changing needs and preferences of the population.
