He does not have any pertinent past medical history. He is married and has five young children. He states he has been having problems for a while but delayed seeing the doctor because of his work schedule and limited money. The doctor performs a digital rectal examination in the office and finds that Jose’s prostate is enlarged. Discuss three potential barriers that you would anticipate when providing care to Jose based on his age and culture. Provide an example of how you would overcome each of the three barriers through the implementation of multidimensional care strategies. Directions:

Potential disease processes and signs/symptoms:
Based on the signs and symptoms Jose is presenting (edema of the scrotum, urinary retention, hematuria), the potential disease process is benign prostatic hyperplasia (BPH). BPH is a noncancerous enlargement of the prostate gland that is common among aging men. As the prostate grows larger, it can squeeze the urethra and bladder neck, causing problems with urination. The signs Jose is experiencing align with the urinary obstruction that results from an enlarged prostate.
Multidimensional care for Jose:
To provide multidimensional care for Jose that addresses his cultural, psychosocial, spiritual, physical, and emotional needs, some strategies could include:
Use a medical interpreter to overcome the language barrier, so Jose fully understands his condition, treatment plan, and can effectively communicate his concerns.
Respect Jose’s cultural values as a Hispanic man. He may feel embarrassment discussing private health issues. Build rapport and reassure him that his concerns will be addressed sensitively.
Address potential financial barriers by connecting Jose to social services programs for which he qualifies to assist with medical costs if needed.
Recognize the stress of Jose’s family responsibilities as a husband and father while dealing with his health issues. Inquire about his support system and provide referrals to community resources if additional support would help reduce his stress.
Perform a spiritual assessment to determine if spiritual practices could provide Jose comfort during his treatment and recovery. Involve his church community if appropriate.
Validate Jose’s emotions regarding his diagnosis and any lifestyle changes needed. Offer counseling referrals if signs of depression are present.

Here are three potential barriers I anticipate for Jose based on his age, culture, and situation, along with examples of multidimensional care strategies to overcome each:
Language barrier – Overcome through use of a medical interpreter as mentioned above. The interpreter can help explain treatment options and ensure informed consent.
Financial constraints – Overcome through connecting Jose to social services programs to assist with costs of care and prescriptions if needed. Community health centers on a sliding scale may also help.
Work/family responsibilities – Overcome by providing flexible appointment times if possible, written care instructions, and referrals to community resources for additional family support to help reduce stress on Jose from competing responsibilities.
