Assessment Description

You will write a series of structured reflective journal entries that focus on your learning and professional development during your community health nursing practicum. Each entry will analyse a specific experience or set of related experiences using an approved reflective model and will link observed practice to community health nursing concepts, professional standards, and relevant evidence.

Task Instructions

Step 1: Choose Experiences for Each Entry

• Select one significant experience for each entry that challenged you, extended your skills, or shifted your understanding of community or population-focused care. Examples include a home visit, screening clinic, health education session, interprofessional meeting, or work with a vulnerable population group.

• Ensure the three entries reflect a range of community health nursing activities, such as health promotion, case management, advocacy, and interprofessional collaboration.

• Maintain confidentiality by omitting or altering names and any identifying details of clients, families, or services.

Step 2: Use a Structured Reflective Model

• Use an approved reflective framework for each entry, such as Gibbs’ Reflective Cycle or a similar structured model.

• Apply the model consistently so that each entry includes the following components:

  • A clear description of the situation

  • An honest but professional account of your thoughts and feelings

  • An evaluation of what went well and what was challenging

  • An analysis drawing on theory, guidelines, or evidence

  • A conclusion summarising learning

  • A specific action plan for future practice

Step 3: Structure of Each Reflective Entry (500–700 words)

Use subheadings aligned to your chosen reflective model.

1. Description (approximately 100–150 words)

   • Outline where the experience occurred, who was involved using non-identifiable labels, and your role as a student nurse.

   • Focus on essential contextual details and avoid unnecessary clinical information.

2. Thoughts and Feelings (approximately 100–150 words)

   • Describe your initial reactions, assumptions, concerns, or expectations.

   • Briefly reflect on how your feelings influenced your communication, decisions, or actions.

3. Evaluation (approximately 80–120 words)

   • Identify what worked well and what did not.

   • Consider the perspectives of the client, family members, community participants, and the healthcare team.

4. Analysis (approximately 150–200 words)

   • Link the experience to relevant community health nursing concepts such as social determinants of health, equity, empowerment, health promotion, or person-centred care.

   • Support your analysis with one or two relevant sources, such as professional standards, practice guidelines, or research literature.

   • Reflect on what the experience reveals about systems, team dynamics, or your emerging professional role.

5. Conclusion and Action Plan (approximately 80–120 words)

   • Summarise key learning points and identify strengths and areas for development.

   • Outline clear and achievable actions to improve future practice, such as skills to practise, feedback to seek, or strategies to apply in similar situations.

Formatting and Submission Requirements

• Total length of 1,500–2,100 words across all three entries.

• Typed, double-spaced, using a 12-point font, with a word count provided for each entry and for the total document.

• Clearly label each entry as Entry 1, Entry 2, and Entry 3, including dates that align with your practicum timeline.

• Write in the first person while maintaining professional language and respect for clients and colleagues.

• Use APA 7th edition for any sources cited within the analysis sections.

• Submit the completed journal via the learning management system by the specified due date.

Marking Criteria / Rubric

Criterion 1: Depth and Quality of Reflection (40%)

High distinction work demonstrates critical and honest reflection that goes beyond description, with clear examination of personal responses, decision-making, and professional responsibilities, and evidence of developing insight across entries. Pass-level work shows some reflective depth but may remain descriptive in parts. Unsatisfactory work is largely descriptive with limited evidence of learning or self-awareness.

Criterion 2: Integration of Theory, Evidence, and Standards (30%)

High distinction submissions clearly link practice experiences to community health nursing concepts, reflective models, and professional standards, using literature to deepen analysis. Pass-level submissions make some connections, though these may be inconsistent. Unsatisfactory work shows minimal or inaccurate integration of theory or evidence.

Criterion 3: Professionalism, Structure, and Writing (30%)

High distinction entries are well structured, coherent, and within word limits, demonstrate confidentiality and professional tone, and apply APA referencing accurately where required. Pass-level work is generally clear with minor issues. Unsatisfactory work lacks structure, contains frequent errors, or demonstrates unprofessional language or breaches of confidentiality.

Analysis and Action Plan

The home visit highlighted how easily time pressures and task-oriented approaches can overshadow the relational dimensions of community nursing practice. The client’s living conditions, family responsibilities, and financial limitations influenced her capacity to follow health advice as much as her clinical condition, yet my initial focus centred primarily on symptoms and compliance. When viewed through a community health nursing lens, this experience reinforced the importance of assessing social determinants and understanding individual priorities rather than assuming education alone will lead to behaviour change. In future practice, I plan to use more open-ended questions and explicitly invite clients to identify their most pressing concerns. I will also seek guidance from my preceptor on incorporating routine prompts into assessment tools to ensure social and environmental factors are addressed alongside physical health needs.

Structured reflective writing plays a critical role in helping nursing students translate practicum experiences into meaningful professional learning. Evidence suggests that using a consistent reflection model supports deeper critical thinking, enhances self-awareness, and strengthens the integration of theory with practice in community settings. For students in population-focused placements, reflection also promotes ethical sensitivity, cultural awareness, and professional accountability by encouraging deliberate consideration of power, context, and client perspectives within real-world health systems (Rolfe, 2019).
