Nurses’ Scope of Practice and Autonomy Under Nurse Practice Acts

Nurses are essential members of the health care team who provide safe and competent care to patients. However, the roles and responsibilities of nurses vary depending on the state they practice in. Each state has a Nurse Practice Act (NPA), which is a law that defines the scope and boundaries of nursing practice. The NPA also establishes the authority and functions of the Board of Nursing (BON), which is the regulatory body that oversees nursing education, licensure, and discipline.

The purpose of the NPA is to protect the public by ensuring that only qualified individuals can practice nursing and that they adhere to the standards of practice set by the BON. The NPA also protects the nursing profession by legitimizing it as a distinct and autonomous discipline with its own body of knowledge, skills, and ethics.

The scope of practice of nurses refers to the range of activities that nurses are legally authorized to perform based on their education, training, and competence. The scope of practice varies for different levels of nurses, such as registered nurses (RNs), licensed practical/vocational nurses (LPNs/LVNs), and advanced practice registered nurses (APRNs). The scope of practice also depends on the state’s NPA, which may be more or less restrictive than other states.

The autonomy of nurses refers to the degree of independence and self-governance that nurses have in making decisions and taking actions related to patient care. The autonomy of nurses is influenced by several factors, such as the state’s NPA, the organizational policies and culture, the collaboration and communication with other health care professionals, and the patient’s preferences and needs.

One of the challenges that nurses face is to balance their scope of practice and autonomy with their accountability and responsibility. Nurses are accountable for their actions and outcomes to their patients, employers, BONs, and society. Nurses are responsible for following the NPA and the rules/regulations of their BONs, as well as the ethical principles and professional standards of nursing.

Nurses can enhance their scope of practice and autonomy by staying updated on their state’s NPA and BON rules/regulations, pursuing continuing education and competency development, seeking certification and specialization in their areas of interest, advocating for their rights and roles in health care policy and legislation, and collaborating with other health care professionals to provide holistic and patient-centered care.
