You will create a portfolio of resources for facilitating programs. The programs will include title, frequency, goals and objectives, skills required, materials or equipment, 2-3 adaptations per program, etc. The following components will be included in your portfolio:
1. Creative Title Page
2. Table of Contents
3. 5 Domains with 2 Activities per Domain
a. Physical (2 activities)
b. Cognitive (2 activities)
c. Emotional (2 activities)
d. Social (2 activities)
e. Spiritual (2 activities)
4. Reference Page

This assignment is an opportunity to creatively present activities. This can be used for your career as your β€œguideline” for programs in each domain. Please include different colours, pictures, etc.

Programs Student includes 2 programs for each domain (10 programs total) Student includes 1-2 programs for each domain. Student includes an inconsistent number of programs for each domain. Student is missing several programs from each domain.
Activity Analysis Student provides excellent analysis of each program and includes all the following: title, frequency, goals and objectives, skills required, materials or equipment, 2-3 adaptations per program, etc. Student provides good analysis of each program and includes all or most of the following: title, frequency, goals and objectives, skills required, materials or equipment, 2-3 adaptations per program, etc. Student provides fair analysis of each program and includes most or some of the following: title, frequency, goals and objectives, skills required, materials or equipment, 2-3 adaptations per program, etc. Student provides limited analysis of each program and includes few of the following: title, frequency, goals and objectives, skills required, materials or equipment, 2-3 adaptations per program, etc.
