Title Slide:
Resolving Ethical Dilemmas: The Role of Advanced Practice Nurses
Introduction (1 slide):
The purpose of this presentation is to examine the contributions of advanced practice nurses in resolving ethical dilemmas in healthcare settings, analyze a specific ethical dilemma through the lens of ethical principles and the ANA Code of Ethics, and provide recommendations for its resolution.
Advanced Nursing Role (2 slides):
Advanced practice nurses (APNs) are uniquely positioned to help resolve ethical dilemmas in healthcare due to their:

Advanced clinical expertise and knowledge (Hamric et al., 2023)
Leadership roles and influence within healthcare teams (Epstein & Turner, 2020)
Close relationships with patients and families (Grace, 2018)

APNs can facilitate ethical decision-making by:

Identifying and clarifying ethical issues (Butts & Rich, 2019)
Advocating for patient autonomy and best interests (ANA, 2021)
Collaborating with the interprofessional team to explore options and build consensus (Ulrich et al., 2022)

Description of an Ethical Dilemma (3 slides):
Scenario: A 72-year-old man with advanced dementia is admitted to the hospital with aspiration pneumonia. He lacks decision-making capacity and does not have an advance directive. His daughter wants all aggressive measures taken, while his son feels comfort care would be more appropriate given their father’s poor quality of life. The healthcare team is divided on the best course of action.
Stakeholders impacted:

The patient, whose autonomy and best interests are at stake
The conflicted family members
The healthcare professionals with differing opinions on the ethics of the situation

Ethical Analysis (3 slides):
Key ethical principles in conflict:

Autonomy – respecting the patient’s wishes, which are unknown
Beneficence – providing care that benefits the patient
Non-maleficence – avoiding harm to the patient (Beauchamp & Childress, 2019)

Relevant provisions of the ANA Code of Ethics:

Provision 1.4 – the right to self-determination (ANA, 2015)
Provision 2.1 – commitment to the patient as the primary responsibility
Provision 5.1 – moral self-respect and preservation of integrity

Recommendations (2 slides):

Arrange a care conference with the family to better understand the patient’s values and preferences, and see if consensus can be reached (Wocial, 2021).
Consult the hospital ethics committee for guidance and mediation (Tarzian & Wocial, 2022).
Aim to reach a proportionate decision that respects the patient’s dignity and balances benefits and burdens of treatment (Jonsen et al., 2022).

Conclusion (1 slide):
Ethical dilemmas are complex situations that require careful analysis and collaboration to resolve. Advanced practice nurses have a vital role to play in identifying issues, facilitating discussions, and advocating for ethically appropriate care. By applying ethical principles and codes of ethics, and engaging key stakeholders, APNs can help navigate challenging cases to uphold the patient’s best interests.
